NC SIP Interactive Online Database - Reading

 

Please use the link above to access the NCSIP database, which will be used to update all site information.  If you have any questions regarding the use of the new database or how it has replaced any of the previously required forms, please contact Jennifer Averitt.  As a reminder, the Fidelity Observation forms ARE NOT currenly a part of the database and the the links below should continued to be used.

 

Please be sure to refer to these INSTRUCTIONS for submitting 2015-16 Student Data, Site Information, Professional Development Events, and Developmental Review via the online database.

 

Please complete this Participants List spreadsheet at the conslusion of each event and upload to the Events Database.

Please use the file upload tool in the NCSIP Online Interactive Database to upload your 2016-17 Student Progress Data Forms and K-2 forms and use the following naming convention:

LEA#_LEA name_2017 Student Data Form (ie: 430_Harnett_2016 Student Data Form)

EFFECTIVE 7/1/15, THE FIDELITY OBSERVATION FORMS INCLUDE AN AUTO CALCULATION FILED THAT WILL AUTOMATICALLY CALCULATE THE FINAL FIDELITY SCORE.  WHEN YOU SUBMIT THE COMPLETED FORM ONLINE, YOU WILL RECEIVE AN EMAIL WITH THE COMPLETED FORM INCLUDING THE SCORE.  PLEASE NOTE THE NEW RATING SYSTEM BELOW.  IT IS CRITICAL THAT YOU INCLUDE THE NUMBER OF RATINGS LEFT BLANK AT THE END OF THE FORM AS PROMPTED!

Expenditure Forms

PRC 082 Expenditure Report will be submitted electronically to your Regional ECD PIPD Consultant by March 31 (mid-year) & September 30 (Final)

As of December 2014, PRC 082 expenditure reports are a part of the combined expenditure reports now and are sent to the LEA EC Director in December prepopulated with the amounts that are allotted to that LEA. Therefore, they are customized and not generic. In the event that you need a copy of your current PRC 082 expenditure report, you may contact Kelly Breest at Kelly.breest@dpi.nc.gov
